What the Hearing and Vision Team do
The Hearing & Vision Team predominantly provides Care and Support Needs Assessments under the Care Act.
They also conduct reviews and support planning for individuals who:
- have a sensory impairment
- have the appearance of care and support needs
- are resident in Bath and North East Somerset
The Hearing and Vision Team also:
- complete ongoing case management (including responding to urgent support needs as and when a person’s situation or needs change)
- conduct Annual Reviews for those open to the team
- undertake relevant Mental Capacity Act assessments and 'best interest' decision making
- offer advice or support for visually impaired adults from a Rehabilitation Officer for Visually Impaired (ROVI)
- offer registration for Hearing and/or Visual Impairment on the Council register
Team structure and funding
The team comprises of a Senior Practitioner and Social Workers. The support they offer is funded by Bath and North East Somerset Council.
