Submit your Infection Control Fund monitoring returns

This application should be completed by someone who is authorised to certify that this money was spent in accordance with the conditions of the grant.

You should use this form to tell us about funding allocated during Infection Control Fund Round 3, and from the Lateral Flow Testing Fund if you are a care home

You should submit your returns by the date shown in the table below.

Get ready to apply

It should take approximately ten minutes to complete the form.

Before starting this form, you'll need to get together everything you need to submit your returns. This includes all of the following:

  • Your CQC registration number
  • The total amount of funding you have received
  • The date you last completed the Capacity Tracker or CQC home care survey
  • The total amount of funding spent in the reporting period
  • The amount and percentage of funding spent on isolating residents
  • The amount and percentage of funding spent on staff movements
  • The amount and percentage of funding spent on paying staff
  • The amount and percentage of funding spent on other infection control measures

Care homes will also need to report on the following measures to show how they have spent Lateral Flow Testing (LTF) fund grants:

  • The amount and percentage of funding spent on training and carrying out lateral flow device (LFD) testing
  • The amount and percentage of funding spent on recruiting staff to facilitate increased testing
  • The amount and percentage of funding spent on the creation of a separate testing area where staff and visitors can be tested and wait for their result
  • The amount and percentage of funding spent on the disposal of LFD tests and testing equipment
